Appointments

  • Patients will be seen by appointment.
  • Every effort is made to stay on schedule, although emergencies sometimes arise. WE RECOMMEND YOU CALL THE OFFICE ONE HOUR PRIOR TO YOUR SCHEDULED APPOINTMENT to assure the doctor has not been called out for a delivery. With respect to patients who have appointments, no walk-ins can be accommodated. Our staff has been instructed to inquire about the nature of your visit to assure sufficient time will be scheduled to care for your specific needs.
  • We will begin a permanent record on your first visit. You will be asked to complete a medical history and a registration form. If you have been under the care of another physician, we may ask permission to transfer some of your records.
  • Appointments can be scheduled up to three months in advance.

Cancellations

  • If you are unable to keep your appointment, or if you are going to be late, please call the office to notify us. This courtesy allows us to be of service to other patients.
  • If you arrive more than 10 minutes late, this may result in a reschedule of your appointment.

Emergencies

  • If you have an urgent problem, please call the office for instructions, 24 hours a day: 419-668-2686.

Telephone Policy

  • Please call our office if you have any questions regarding your condition, medication or treatment. All return calls are scheduled to be made by late morning and late afternoon.
  • You will be notified of laboratory results by phone when all studies have returned.

Payment

  • Payment at the time of service is requested. For your convenience, Mastercard, Visa, Discover and personal checks are accepted.
  • If you have insurance, you need to bring your ID card(s) with you to each visit. We will file your forms promptly and request you pay your portion when services are rendered.
  • If you need to arrange a payment plan on surgical or delivery fees, we will assist you on an individual basis.
  • If you have any questions or need for clarification regarding our services or fees, please do not hesitate to ask.

Insurance

  • We are a participating provider in most insurance plans serving this area. For clarification on a specific plan, please feel free to call our office. We accept most types of insurance as well as Medicare and Medicaid.
  • Each insurance company has different coverage guarantees. Usually, no two insurance companies are alike regarding what they will cover in terms of medical services. It is a good idea to contact your insurance company to determine what your policy will cover.
  • Our policy regarding insurance is that you are responsible for your co-payments and deductibles at the time of service. We will be happy to submit your insurance claims for you.
  • If you have any questions regarding your insurance claims or status of your account, please do not hesitate to ask.

Laboratory

  • Following current trends in laboratory care, your insurance company may have contracted with a specific laboratory facility (i.e. LabCorp, LabOne, Universal Standard, etc.). It is your responsibility to be aware of the laboratory requirements mandated by your insurance carrier.
  • By providing our office with this information, we will be able to assist you in attaining the highest level of benefits available to you for your laboratory services.

Pregnancy Tests

  • The need to know now is every womans concern. Consequently, pregnancy tests are available any time during office hours for a fee of $5.00.

Confidentiality

  • The confidentiality of medical records and all conversations with our office are assured. We will not give out information regarding your condition to your employer, friends or relatives without your permission.

Office Policy

  • We feel it is important for patients and their families to understand as much as possible about their care. We make a special effort to explain everything to you regarding your condition, treatment and medicine. If you ever have questions, or if something is not clear, please do not hesitate to ask.
  • We have chosen our personnel, office procedures and medical equipment with great care to provide quality medical services in a pleasant, efficient and friendly atmosphere. If you have any suggestions or complaints regarding our services or fees, please tell us so we may better serve you.
